Aging in Place Planning Workbook
My wishes, my people, my home, my plan.
Plan to stay home, in your own words
Most people want to age in their own home. Fewer get to, because when a crisis comes no one has the authority, the instructions, the money or the support to bring them home. This workbook helps you put those pieces in place while you can still make every decision yourself.
It is a plain-language guide with places to write: your wishes, your decision-makers, your documents, your health and daily living, your home, technology, your care team and family, paying for care, protection against exploitation and isolation, and what to do if a facility ever becomes necessary.
October 2026 edition · about 26 pages · fillable on your computer, or print it and use a pencil.
